Description of Incident

A satellite antenna fell from the mounting point on the vessels monkey island to the deck below. The distance was approximately 6 metres, and the weight of the antenna was approximately 3 kgs.

The Drops calculator has been used to plot the potential outcome of this as shown.

An investigation has revealed that the satellite antenna was mounted on a foundation with rubber compensators like the example shown below. Over time the rubber was not strong enough to hold the antenna and subsequently failed.

The antenna fell down during a period of severe weather whilst the vessel was at sea. The rough weather may have been the final cause for the rubber compensators to break.
